Yup. years ago, on the even of the microprocessor age. Designing
high performance stepper motor translator. running amplifier from
the lab's heavilly filtered 100 volt DC bus backed by a 500 amp
supply. Using a shift register to implement the switching regime.
Occasionally the shift register would load wrong and crowbar the bus
with one side of the H driver. After the smoke cleared, one would
observe the exact outline in the top of the TO-3 can of the pins,
the bonding wires and the die. Exploding wire cutting at its finest
:-) Of course, the high speed semiconductor fuses just sat there
and watched it happen.
