That is why I always use a bang stick to short the capacitors in addition to bleeders before I touch anything just in case.

Yes a failed bleeder is a real risk with potentially nasty consequences

In high energy situations I put 3 separate chains across the cap system 
bolted directly to the terminal to safeguard against a bleeder resistor 
failure as a little extra insurance. Recommend more than a single bleeder 
chain myself.
