[TCML] hot-streamer DVD navigation

Hi

For those who have gotten hot-streamer DVDs and copied the files to their hard 
drive---

I have put up a zip archive of "fixed" files that work in a file system.

Not perfect, some links may send you to a folder rather than the index file, but 
pretty close anyway.
