Yes, you can use 3mm od (That's like AWG 8 or 10).   2-3 cm gap from 
secondary to inside of  primary is probably a good value.  If you've 
calculated your primary to be, say, 10 turns, and you're putting 20kV peak 
across it, you'll have about 2kV/turn, which means that you should make the 
spacing between turns at least 3 or 4mm (that's edge of wire to edge of 
wire.... center to center would be 3mm more...).  More likely, your turn to 
turn spacing will be determined by how you're attaching the tap.  Remember 
you want that 3-4mm clearance between the tap/cliplead/etc, and the 
adjacent turns...   A spacing of 10mm wouldn't be a bad idea.
