Just a wire from the top winding of the coil to the metal ball.  The ball
can be screwed, glued, are just set in place.  Often the secondary wire is
just left long to connect to a top terminal.

You need a cap that can withstand 21,000 Volts at very high currents.  Most
people get them from the Geek Group:

The chart there shows how many you need given your NST voltage, current,
and gap type.  Assuming you have a 15000 volt 30mAmp NST and a static gap,
you would want 8nF which is a single string of 19 of their caps:

Use the LTR values since they are the safest for the NST.
