The above comment reminded me of Charles Brush which I believe was the

I seem to recall Jeff Parrise (spelling?) of KVA Effects saying that he
killed one a few years back but that it may have been a "lemon". I
think it was on this list in response to myself bringing up this same
question. Maybe a check in the archives would bring it up? Maybe
Jeff could clarify? Then agian, that may not qualify as "recently"
