The main problem is probably with your caps. In the pictures, the caps just
don't look good. I would abandon placing a cap on each leg of the high side
of the power supply, and rather place a 0.02uf cap on only one leg.  Also
the power supply is perhaps too small to have breakout on a nine inch
diameter secondary with the toroid in place. Because those chokes may not
save your transformers, you should place a safety gap across your power
supply.
