I've used sizes up to 48", it does come bigger.

It's pretty expensive in the larger diameters (many tens of $/ft), but if
you are pouring a bunch of piers for a construction project, the cost of
the form is insignificant, relative to digging the hole for the tube, and
the cost of the engineer to design the system. That is, there isn't much
pressure on the mfr to reduce the price.
