One end of the primary is hooked to one end of the cap.  The other end of
the primary is hooked to the gap.  The other end of the gap is hooked to
the other end of the cap.

The bottom end goes to a really good earth ground.  The top end is
connected to the top terminal.

Very roughly, about 15000 volts depending on your transformer's specs.

The primary coil, cap, and spark gap form a circular loop.  The transformer
charges to cap until the voltage across the gap reaches the point where it
fires.  Put the transformer across the gap.

This is short and sweet, but hopefully specific...
