OK. Certainly the voltage on the switched end of the inductor rises to twice the supply voltages [losses neglected] and the cathode of the diode is hooked to the output capacitor charged to twice supply voltage with respect to ground. When the switched end of the inductor is at ground the reverse voltage across the diode will be the capacitor voltage which is indeed twice the supply voltage.

I've never understand where the 'De-Quing notion came from because there's no Q involved.

  Depends on where you put the diode but and what you mean by 'experience the full swing'.  Circuit?
