You are probably referring to feed-through insulators, which come in mating
pairs designed to run high voltage DC or RF through a panel.  You could
still use them as a standoff by using a nylon or fiberglass bolt.

A lot of the ceramic stand-offs I see do not have threads - which means
my only real option it to run a metal bolt through the middle - doesn't
this defeat the purpose of the stand-off?
