If your NST has one side tied to case, you can't use a 3 terminal gap.
But regardless, I don't think the 3rd-wire ground should go to the NST
at all.

When you say there one side of the secondary tied to the mains ground,
you mean tied to the case, right?  If the NST has a 3-wire power cord,
just disconnect the ground wire from the case, and don't use the 3rd
wire at all.  Connect the RF ground to the case, and use a 2 terminal
safety gap between the case and HV bushing.
