Terry,
I posted this same question to the list a few months ago and I think the
general concensus was that most of these shaft couplings are custom
machined. I too, have not been able to find anything like this from the
typical parts suppliers (not even McMaster-Carr, Grainger, ect). I'm
sure that some of the more mechanically inclined list members would
be willing to make you one for a nominal fee ;^)))
