I ran across a fascinating document from Southern California Edison:
The Electrical Services Requirements (ESR), which is one of the SCE
Distribution Manuals.

It's 300 pages of SCEs requirements for how the service entrace is done,
grounding, how power is supplied, etc. The audience is people building
buildings where Edison is going to supply the power, so they cover how the
meter is to be installed, what services are available, all the grounding
schemes they use, etc.
