what failed was the hi pressure hi volt contact switch...   one of the 
electrodes that pass thru the glass envelope must have heated too much and 
caused a joint failure at the metal/glass connection.  The electrode blew 
out of the envelope leaving a clean hole the same size of the electrode 
along with some fractures emminating from the hole...
