Brent Turner's "The Tesla Coil Book: How They Work And How They Are Built"
comes to mind. I got my copy back in 1997, don't know if it's still in
print. Covers all 3 types and is more than just a set of plans that you find
on ebay (can't believe people actually buy those, when they can get the book
and write their own plans). Has examples and references in the back as well.
