Hi David,
it might be the following paper:
David H. Sloan, A Radiofrequency High-Voltage Generator
Physical Review, Volume 47, January 1, 1935, pp.62 - 71
While this is not one of the Hi-Tech approaches of today, it's a very
interesting description of the CW excitation of a resonator, on the base
of vacuum tubes. The single tuned-circuit resonance transformer should be
superior to the coupled tuned-circuit classical Tesla-coil.
