Yes, connect the ground screw to the earth ground.

No, this is not especially harmful. When the safety
gaps fire, they will clamp the voltage to zero for an
instant, whether the ground lug is earthed or not. But
earth the ground lug on your NST anyway.

This isn't good. No saprks should be coming from the
windings on the secondary. This is usually a symptom
of over coupling. You can cure it by elevating the
seconday a little--perhaps an inch at a time until the
spurious sparks quit.
