Kevin,

Under certain conditions, the voltage stress developed across the coil
winding exceeds the voltage standoff capability of the winding. This
usually happens when the degree of coupling between the primary and
secondary (the coupling coefficient - k) is too high. This can usually
be corrected by raising the secondary above the primary a bit.
