The 4.5 h/d ratio insures the coil has a lot of turns on it, typically 1,200
to 1,400 range.  This gives a higher inductance, and particularly in the
small coils, produces greater potential on the sec coil as the inductance
goes up as the square of the turns and also the square of the dia. of the
coilform.

If you get too many turns you begin to hit a wall due to higher resistance,
but with 30 and 60 mA coils 1,300 turns is a good number to shoot for.

Long 4.5 h/d ratio and also large sec dia.
